6th graders need:  a physical, immunizations (Hep B, Varicella, Meningitis, and Tdap), a dental exam. All forms should be turned in with registration.  These are all due by Monday October 17, 2022.

7th graders need: a sports physical if trying out for any sport.  All vaccines need to be up to date (see 6th grade requirements).

If your student will be taking medication at school; inhaler, epi pen, over the counter and or prescription medications you and your physician will need to complete this form (please note their are two forms)
Medication Authorization Form
Parent/Guardian Authorization for Self Administration
Students are not allowed to carry medication or keep it in their lockers except for inhalers and epi pens with the proper paperwork in place (self administration paperwork)
