Individual Health Care Plans: 
• Chronic health problems such as allergies, asthma, diabetes and seizures, to name a few, may necessitate the development of a care plan to address student medical needs in the school setting. The development of such plans will be based on physician authorization and involve administration, the nurse, teachers, parents, and the student when appropriate. Parents who feel their child may be a candidate for such a plan should contact the nurse’s office in their child's school.

Medical Reviews:
The district Certified School Nurses are an integral part of your child's educational team.  It is critical for us to have the most up to date medical information as we assess their medical and educational needs. 
Students with Individual Educational Plans (IEP) and or 504 Plans will have annual meetings with the educational team.  Every 3 years a full medical review will be completed for those with IEP's with an annual review to make sure everything is going well. 
Students with 504 plans will have annual reviews.  If there are changes in your child's medical care or routine, please let us know.
