All kindergartners are required by the State of Illinois to submit:

• Dental Form Spanish

• Physical Exam and Immunization Form

• Lead Questionnaire

DENTAL exam for 2nd grade is a state requirement.

​If your child brings donated clothes home, please launder and return them to the Health Office.

Lice: It's a great idea to wear your hair up (pony, pigtails, braids). Keeping your hair off of others helps prevent the spread of head lice. If Lice or nits are found, the student will be sent home for a treatment. Please come to the health office for a re-check when nit-free.

Student Athletes:  All athletes Grades 6-8 are required to provide a recent physical.  Physicals are good for 13 months and must be renewed annually.
