Welcome to District 56!

For all students new to the District, the following documents must be provided to the district in order to finalize the enrollment process:

• Birth Certificate
  Families must provide a legal birth certificate to register their children.
  Hospital/baptismal certificates are not acceptable.
• Health Requirements
  Most recent physical examination and current immunization records are required. Kindergarten & 6th grade students require a physical examination completed within one year of school entry.
• Proof of Residency

  THREE documents are required to verify residency.  You must submit proof of residency within District 56 boundaries by providing ONE document from Category 1 and TWO documents from Category 2.  All documents must be current (within past 30 days) and include parent/guardian name and address.
• Illinois Student Transfer Form
  If a student is transferring from another public school in Illinois, an Illinois Student Transfer form must be obtained from the previous school and submitted along with the other registration documents. 

Every attempt should be made to pay school fees at the time your registration is finalized or as soon as possible.  Each student will be assigned an iPad that will be his/her primary instructional tool.  I strongly recommend that each family make accommodations ($30.00) to allow students to have 24-7 access to their iPads to ensure maximum learning gains for them.
