Dear Parents:

 

Enclosedare the documents needed for the registration of your child in Gurnee SchoolDistrict 56.  Please review and correctall pre-printed information as needed. Corrections should be made in blueor black ink directly on the enclosed registration form.

 

Againthis year, District 56 is encouraging allreturning students to mail the completed registration forms in the enclosedself-addressed envelope.  Thedocuments that need to be returned in the enclosed envelope are the correctedStudent Information sheet, payment of fees, and the Student Health/MedicalHistory form.  Homeowners will alsobe allowed to send in the Proof of Residency Check Sheet for Homeowners alongwith copies of their proof of residency. You are not required to provide this information in person.

 

Thecollection of fee payments is a critical aspect of the District’s planning andpreparation for the 2010-2011 school year, and as such, the District will offera $20.00 discount to all families who return their student information sheet,student health/medical form and pay their fees prior to May 14, 2010.  Registration deadline for returningstudents will be June 9, 2010.  Onand after June 10, 2010, a late registration fee of $25.00 will be assessed.  Kindergarten screening and registrationwill be held on June 8 – 9, 2010.  By mailing your registration materialsto the District Office immediately, you will save money, save time, and enjoythe benefits of a returning student.

 

Thosefamilies who reside in the District with leases, or need to complete anaffidavit, will need to personally provide to the District Office staff the original documentation of their proofof residency on or after August 9, 2010. If you register by mail prior to June 10, 2010, you will be provided anopportunity to complete this task in an expedient manner.  Please refer to the Proof of ResidencyCheck Sheet for Homeowners to establish the types of proof you will need tobring with you.  Those requiringaffidavits will need to obtain the form on the District’s website or at one ofthe school offices.  The DistrictOffice has reserved four dates in which to bring in proof of residency.  These dates are August 9 and 12, from 2:00p.m. until 6:00 p.m. and August 10 – 11 from 12:00 p.m. until 4:00 p.m.

 

Complyingwith the request after August 12, 2010, will result in long lines and thepossibility of the student not starting school on time. If all registrationsteps are completed in a timely manner, teacher assignments and transportationinformation will be shared with parents on August 16, 2010.  In order to allow principals anopportunity to prepare class lists; the transportation department to assign busroutes; and teachers sufficient time to prepare for their students; allregistration requirements must be completed on or before August 18, 2010.  Studentsnot registered for school by the end of the day on August 18, 2010, will not bepermitted to start school on time (August 24, 2010) and will begin school on orafter August 26, 2010.
